This project is completely led by our Youth Advisory Council.

Support our Youth Advisory Council (YAC) in their Talent Video project! This year our YAC is putting together a video with all the talents of children and youth with a facial difference. The YAC are looking for:  

• Children and youth ages 6-17 with a facial difference who are  
• Canadian residents showing  
• Any talent at any skill level. If you are learning a new skill and still feel like a beginner, that’s ok! We still would love to see it.

The information in this form is confidential. Since we only ask for a picture of the talent, this form will help us gain a better understanding of how the final video will be filmed. The information in this form does not affect whether your child’s talent is chosen or not. Once we review all the submissions, we will contact you with next steps. Note that not all submissions may be chosen, but we will contact you either way.
